Online Registration Form for Emotional Assessment and Services
This project is fully funded by the Government. Registration and participation in the services are free of charge. Individuals aged 18 or above are eligible to undergo emotional assessment and register for services. For further services and follow up, participants must register as members of the Yau Tsim Mong District Health Centre.
When you submit this form, your personal details (such as name and email address) will not be automatically collected unless you choose to provide them. For any enquiries about the project, please contact the staff of the Yau Tsim Mong District Health Centre at 2326 7800.

If you are interested in this programme, you are encouraged to register as a member of the Yau Tsim Mong District Health Centre, where our staff will first provide you with a comprehensive health risk factor assessment. Due to the current high demand for services, waiting time may be required. We sincerely appreciate your understanding and patience.
This service is primarily designed to provide low intensity mental health support for individuals with mild to moderate symptoms of depression or anxiety.
