Program Content
The Patient Empowerment Program (PEP) is a self-health management support service designed for patients with chronic conditions. Its aim is to enhance patients’ understanding of their diseases and improve their disease management skills, thereby helping them establish healthy lifestyle habits. The Yau Tsim Mong District Health Centre, in collaboration with the Hospital Authority, provides guidance and support for diabetic or hypertensive patients referred from the Hospital Authority’s general or specialist outpatient clinics. Support is delivered by registered nurses and dedicated healthcare professionals through empowerment classes and individual health management follow-ups.
Class Topics (4 sessions, each 90 minutes)

Session 1: Understanding Diabetes / Hypertension
Learn about the characteristics of diabetes and hypertension, as well as related self-monitoring techniques. -
Session 2: Nutrition Management
Understand different nutritional values, including carbohydrate counting and the glycemic index, to help control diabetes or hypertension through diet. -
Session 3: Exercise Management
Learn how exercise affects blood sugar and blood pressure, explore and practice suitable types of exercise, and understand related precautions. -
Session 4: Mental Health
Explore the relationship between diabetes/hypertension and emotions through interactive sharing, and learn simple relaxation techniques.
