Aim
· Introduction of Hypertension and other cardiovascular disease
· Raise awareness on self-care, maintain healthy living style and dug knowledge
· Regular measurement of blood glucose level and maintain proper record, early identification of complication symptoms and formulation of heath management plan
· Emotional support to service users and carers
Service Target
· User newly diagnosed with Hypertension through DHC Express network Doctor
· Known Hypertension patients
Service scope
· Health assessment conducted by Care Coordinator with follow up services
· Formulation of individualized health management plan in accordance to risk level
· Health Coaching
· Participate in Hypertension management class or (Patient Empowerment Programme)
· Regular meet up (face to face, video call or phone call)
Service Charge
The programme is free of charge
