\nAll members are welcomed to join series of free heath promotion activities<\/span><\/strong><\/em>, with topics include:

\n\u00b7 Smoking and drinking related harms
\n\u00b7 Maintain good mental health
\n\u00b7 Pain management
\n\u00b7 Drug management
\n\u00b7 Disease prevention
\n\u00b7 Carers support
\n\u00b7 Other seasonal health topics<\/p>\n
\nFree health risk factor assessment:<\/span><\/strong><\/em>
\nHealth risk factor assessment will be conducted by Care Coordinator or other allied healthcare professional, with basic physical check-up, review of living style, daily mobility, mental health assessment, etc. According to the result of assessment, the Care Coordinator will formulate individualized health management plan towards risk factor identified and suggest to attend different health education activities in order to maintain good health. Chronic disease screening may further be offered whenever necessary.<\/p>\n
\nFor those with confirmed diagnosis of Low back pain or OA Knee, specific chronic disease management programme will be offered for continuous care and health monitoring.<\/p>\nPain Management Programme<\/a><\/h2>\n
