- \u00a0In order to build a good doctor-patient relationship, encourage Scheme Participants’ active engagement in the treatment process and follow the advice of their family doctor to achieve better health outcomes and status, the scheme has built-in collaborative doctor-patient partnership incentive mechanism.<\/li>\n
- \u00a0Incentive is applicable for Scheme Participants with hypertension \/ diabetes mellitus who have been admitted into the treatment phase.<\/li>\n
- \u00a0All eligible Scheme Participants will be automatically enrolled in the incentive scheme upon entering the treatment phase.<\/li>\n
- \u00a0Incentive payment calculation will start from the second Participant Programme Year onwards.<\/li>\n
- \u00a0After achieving the incentive targets, Scheme Participant can enjoy a reduction of Medical Consultation Co-Payment fee up to the Government recommended Co-Payment amount of $150 for the first subsidised consultation in the following Participant Programme Year.<\/li>\n
